The services that will be provided for your loved one will be outlined in a Care Plan to which you agree. This Care Plan will be followed by the caregiver, whether care is provided on an hourly or live in basis. In addition, the caregivers’ activities are overseen on a regular basis by Elder Bridge’s Registered Nurses and tracked weekly by reviewing the caregivers’ care notes on a weekly basis.
